Reporting Instructions for Completing the GFWC Reporting Form
GFWC Reporting Form | Reporting of Fundraising on the GFWC Report Form
1. Complete the first page of the form, providing all requested information.

Check the year submitted.

List the club name and number of members.

Check General Club, Junior Club, or Juniorette Club.

Indicate the report preparer’s name, address, phone number and email address. This is needed in case the GFWC Reporting Form is detached from the GFWC Virginia Narrative Report Form and/or cover sheet and if additional information is needed by the District or State Chairman.
2. Total the statistics for all reporting areas and enter the information on the GFWC Reporting Form.

The Number of Projects is the numeric account of the respective programs and projects initiated by and participated in by the club.

The Number of Members Participating is the numeric account of the members who took part in the respective programs and projects. The total should not exceed the number of dues paying club members.

The Volunteer Hours are the hours that club members worked on the respective programs and projects.

The Dollars Donated is the monetary amount donated to organize and fund the respective programs and projects.

The In Kind Donations is the monetary values for goods provided.
3. For each narrative report submitted, mark an “X” in the “Narrative Enclosed” column.
4. Attach one (1) copy of the completed GFWC Reporting Form to each completed GFWC Virginia Narrative Reporting Form to be submitted. Highlight the line of the GFWC Reporting Form that corresponds to the GFWC Virginia Narrative Reporting Form and submit it to the appropriate DISTRICT CHAIRMAN.
INSTRUCTIONS FOR MAILING REPORTS
1. Use the GFWC Virginia Reports Distribution Matrix posted on this website for all mailing instructions.
2. MAIL CLUB REPORTS TO THE APPROPRIATE DISTRICT CHAIRMAN, not to the State Chairman.
